• Copies of Pathology Reports
• Allergies
• X Rays, MRI and CT Scans
• Employers Letter (if Workcover)
• Private Hospital Insurance Details
• Medicare, DVA or Pension Card
• List of current medication
• Drivers Lincense or valid ID
